Form 9465 Part I must be completed by all applicants. Be sure to identify what type of tax return (Form 1040, Form 941, etc.) the application is for and the tax year or years.

Lines 1-4. In this section, enter your contact information and Social Security Number (SSN). If you are a sole proprietor whose business is defunct, you must complete Line 2 and also enter your Employer Identification Number (EIN).

Line 8. Enter the value that you will append to the form. To limit your penalty and interest charges, pay as much as possible upfront.

Line 11a. Enter the amount you can pay monthly for ALL your tax obligations, including an existing installment agreement. If you leave this field blank, your payment will be calculated by dividing your balance due by 72 months.

Line 11b. If the amount on line 11a is less than line 10 and you can increase your payment amount to the same amount or more than the amount on line 10, write the changed amount on line 11b. If the amount on line 11a is greater than line 10 and your tax balance is between $25,000 and $50,000, you do not need to fill out Form 433-F. However, you must complete line 13 or 14. Form 433-F is required for tax balances (line 9) greater than $50,000.

Line 13 (a-c). If you wish to make direct debit payments from a checking account, enter the bank code in 13a and the account number in 13b. Low-income taxpayers who wish to receive a usage fee refund and are unable to make a direct debit, please check box 13c.

When the form is complete, staple it to the front of your tax return and mail it to the address listed on your tax return booklet.
